          <dc:description xml:lang="el">Η ανοσοθεραπεία με αναστολείς σημείων ελέγχου της ανοσίας έχει μεταβάλει ριζικά τη θεραπευτική προσέγγιση του μη μικροκυτταρικού καρκίνου του πνεύμονα. Παρά τα σημαντικά κλινικά οφέλη, ένα μεγάλο ποσοστό ασθενών εμφανίζει πρωτοπαθή ή επίκτητη αντοχή, περιορίζοντας τη μακροχρόνια αποτελεσματικότητα των θεραπειών αυτών. Η παρούσα βιβλιογραφική ανασκόπηση συνοψίζει τους βασικούς μηχανισμούς ανοσοαντοχής και τις σύγχρονες φαρμακολογικές στρατηγικές υπέρβασής της. Οι μηχανισμοί αντίστασης είναι πολυεπίπεδοι και περιλαμβάνουν τόσο ενδογενείς παράγοντες του όγκου όσο και χαρακτηριστικά του μικροπεριβάλλοντος (TME). Ενδογενώς, η χαμηλή ανοσογονικότητα λόγω μειωμένου φορτίου νεοαντιγόνων, καθώς και διαταραχές στην αντιγονοπαρουσίαση οδηγούν σε μειωμένη αναγνώριση από το ανοσοποιητικό σύστημα. Παράλληλα, η εξάντληση των CD8+ Τ λεμφοκυττάρων με την υπερέκφραση ανασταλτικών υποδοχέων συμβάλλει σημαντικά στην αποτυχία της ανοσολογικής απάντησης. Το TME διαδραματίζει επίσης καθοριστικό ρόλο μέσω της δράσης ανοσοκατασταλτικών κυττάρων. Επιπλέον, η υποξία και η αγγειογένεση ενισχύουν την ανοσοδιαφυγή και την ανάπτυξη ανθεκτικών φαινοτύπων. Στο θεραπευτικό πεδίο, αναπτύσσονται συνδυαστικές στρατηγικές με στόχο την υπέρβαση της αντοχής, όπως διπλός αποκλεισμός σημείων ελέγχου της ανοσίας, συνδυασμοί με αντι- αγγειογενετικούς παράγοντες, antibody-drug conjugates, διειδικά αντισώματα, καθώς και αντικαρκινικά εμβόλια και κυτταρικές θεραπείες. Οι προσεγγίσεις αυτές επιδιώκουν την ενίσχυση της Τ-λεμφοκυτταρικής απόκρισης και τη μετατροπή ανοσολογικά “ψυχρών” όγκων σε “θερμούς”, με στόχο τον περιορισμό της αντίστασης.</dc:description>
          <dc:description xml:lang="en">Immune checkpoint inhibitor-based immunotherapy has fundamentally transformed the therapeutic landscape of non-small cell lung cancer. Despite the substantial clinical benefit, a significant proportion of patients develop either primary or acquired resistance, thereby limiting the long-term efficacy of these agents. This narrative review summarizes the principal mechanisms underlying immunotherapy resistance and outlines current pharmacological strategies aimed at overcoming it. Resistance mechanisms are multifactorial and involve both tumor-intrinsic factors and features of the tumor microenvironment (TME). At the intrinsic level, reduced tumor immunogenicity due to a low neoantigen burden, as well as defects in antigen processing and presentation, lead to impaired immune recognition. In parallel, functional exhaustion of CD8+ T lymphocytes, characterized by the upregulation of multiple inhibitory receptors contributes significantly to ineffective antitumor immune responses. The tumor microenvironment plays an equally critical role in promoting immune escape. The presence of immunosuppressive cellular populations in the TME, actively suppresses cytotoxic T-cell activity. Moreover, hypoxia and aberrant tumor angiogenesis further enhance immune evasion and support the emergence of resistant tumor phenotypes by altering immune cell infiltration and function. From a therapeutic perspective, multiple combinatorial strategies are currently under investigation to circumvent resistance. These include dual checkpoint blockade, combinations of immune checkpoint inhibitors with anti- angiogenic agents, antibody–drug conjugates, bispecific antibodies, as well as cancer vaccines and adoptive cell therapies. Collectively, these approaches aim to restore and amplify T-cell–mediated antitumor immunity.</dc:description>
