The article provides suggestions for the management of incidentalomas
based on the author’s experience with treating surgically different
adrenal diseases in 217 patients. The main goal in evaluating these
tumors is to rule out subclinical Cushing’s syndrome, subclinical
pheochromocytoma, subclinical primary aldosteronism, primary adrenal
carcinoma, and solitary adrenal metastases. When evidence of subclinical
function or potential for malignancy exists, then laparoscopic
adrenalectomy is indicated for definitive diagnosis and treatment.
Otherwise, follow-up for several years with repeated hormonal and
imaging studies are necessary to confirm the secretory and benign nature
of these lesions.
