Summary:
Objective: The high prevalence of mental disorders during pregnancy and
childbirth leads to the creation of special women's mental health centers
worldwide, which among other things provide specialized health services for
women with mental illness during the perinatal period.
The aim of this study was to assess the prevalence, demographics and risk
factors of perinatal mental illness in women examined in the section of women's
mental health at the University Clinic of Eginition hospital since its
inception in 2003, until 2012. It is the first public special section of
women's mental health in Greece.
Material - Method: We studied the data of 250 women examined in this section in
the decade from 2003 to 2012, based on the files and folders in the clinic. We
describe the demographic characteristics, mental illness, the course, response
and outcome of treatment. Furthermore, in 55 of the 126 women exmanined with
mental illness during pregnancy, a telephone interview with a questionnaire
created by the authors was performed, in order to investigate the present state
of health, information regarding the labor, family history and the current
children's status.
Results: Mean age of these women was 33.18 years. 81.2% of women were married,
while 59.6% of women had had primary or secondary education. Almost half of the
patients had symptoms of mental illness during pregnancy, slightly smaller
number of patients were first examined in the puerperium. In 50% of women there
was a history of mental illness before pregnancy. 68.8% of women received
medical treatment, 27.6% psychotherapy. Of the women who completed the
assessment, in 88% the outcome was good or very good after treatment.
The telephone interview revealed that 45.5% of women received treatment again
after the perinatal period, while 43.6% of women were under treatment during
the time of the interview.
54.5% of women gave birth in a private clinic, while 16.4% gave birth by
caesarean section due to mental disorders, primarily as a woman's desire. 18.2%
of children were born prematurely and 27.3% of women did not breastfeed because
of their mental condition after childbirth or due to medication incompatible
with breastfeeding. 41.8% of women had a positive family history of mental
illness and 8 out of 55 women reported a greater or lesser child behavior
disorder.
Conclusion: Mental illness of women in Greece during pregnancy and postpartum
except for the effects on the overall health of the woman also influence
important decisions such as the type of childbirth, and breastfeeding. An
important role in pregnancy is displayed by preexisting mental illness, and in
turn perinatal mental disease seems to result in high rates of disease after
the perinatal period. The study highlights the role of the first special
women's mental health department in Greece as a basis for treatment and
long-term monitoring of women's mental health in a specific tertiary center.
