Summary:
Background: Malignant mesothelioma can appear up to 40 years after exposure to
asbestos and its differential diagnosis from metastatic adenocarcinoma in
pleural biopsy presents particular difficulties in some cases, even when using
immunohistochemistry.
Objective: To investigate whether classical biochemical markers of pleural
fluid and serum can be used in the differential diagnosis of malignant pleural
mesothelioma from metastatic adenocarcinoma, in patients with pleural effusion.
Materials, patients and methods: Retrospective study from the electronic
registry of the oncology unit of "Sotiria” Chest Hospital. During 2001-2011, 15
files of patients with malignant pleural mesothelioma and 33 files of patients
with adenocarcinoma and pleural effusion were found to offer sufficient data.
Results: Marginally statistically significant difference was observed in serum
LDH value, while statistically significant difference was found in glucose
levels of pleural fluid (p=0.005). No patient in the group of mesotheliomas
showed pleural fluid LDH / LDH serum ratio of less than 0.94. The difference in
the proportion of patients with values below the cut off point proved
statistically significant between the two groups of tumors.
Conclusion: The above findings are likely to help in differentiating metastatic
lung adenocarcinoma and malignant pleural mesothelioma in cases of ambiguous
histology or contraindication to thoracoscopy.
