Introduction: Acute appendicitis is the most common surgical condition of the
abdomen in children and adolescents and is this condition which arises
frequently misdiagnosed. Acute appendicitis is a histopathologic diagnosis. It
is a condition that most often affects the white race and even during the
summer months. The diagnosis is often difficult because of anatomical
variations of the position of the appendix, and a multitude of diseases with
similar clinical picture. The aim of this prospective study was to investigate
the possible association of suPAR with other inflammatory markers such as
CRP, white blood cells and neutrophils in children with clinical possible acute
appendicitis, and correlation scale Alvarado with suPAR. Material - Methods:
We studied 84 children aged from 12 months to 16 years, 54 of them visit the
emergency department of G.N.P. Athens, "Aghia Sophia" with clinical
manifestations of acute abdominal pain, 30 children who visited for planned
surgery for another condition non-inflammatory origin and constituted the
control group. The study lasted from June 2011 to March 2013.'s Clinical input
data included age, weight, time of onset of symptoms, complete blood count,
CRP, suPAR, recording scale Alvarado, the ultrasound data and the
pathology report of the surgical preparation of the appendix . Results: it was
found that there is a statistically significant correlation scale Alvarado with
suPAR (p = 0,000), took control of suPAR with ROC and calculated the AUC
(AUC = 0.77 with p <0,05), the optimum value of suPAR found 2 35ng/ml with
0.1 specificity and sensitivity of 0.6, the estimated OR = 6.85 and adjusted for
age and sex OR = 11,56 confidence interval CI = 2,78-48,1 with p = 0,001.
The positive predictive value of 91.43% and a negative predictive value of
55.10% for suPAR, while the test sensitivity of 59.3% and specificity 90%.
Conclusion: The results suggest that suPAR can help in the diagnosis of
acute appendicitis, but needed more clinical data to find better clinical
results
for the use of suPAR in children with acute abdominal pain
