Background – Aim of the study: Lung cancer is the leading cause of global
cancer death with oxidative stress playing a significant role in its
pathogenesis. 8-isoprostane is an established biomarker of oxidative stress
that is significantly involved in lung cancer pathogenesis. Aim of the current
study is to evaluate the effect of chemotherapy at the levels of oxidative
stress as expressed through the measurement of 8 isoprostane serum levels of
patients with Non Small Cell Lung Cancer (NSCLC). Moreover correlations with
both clinical and pathology parameters were evaluated.
Patients and Methods: Forty four patients with inoperable non-small cell lung
cancer were initially enrolled in this study and twenty nine of them were
finally evaluated in regards to 8-isoprostane levels in serum before and after
chemotherapy (after completion of 6 cycles of chemotherapy) and in correlation
with the extent of the disease and response to chemotherapy. Moreover thirty
healthy volunteers were also enrolled as controls.
Results: 8-isoprostane serum levels were statistically significant lower after
chemotherapy (p= 0.05). Further analysis showed that the above differences were
mainly attributed to the extent of the disease, as well as to the response to
treatment. More specifically patients diagnosed with limited / locally advanced
disease (stages IB – IIIB) presented with post chemotherapy serum 8 -
isoprostane levels statistically significant lower (p=0.09) , furthermore
patients evaluated with partial response (PR) after chemotherapy also presented
statistically significant lower serum 8 – isoprostane levels ( p= 0.07).
Conclusion: In this prospective study - with the limitation in the number of
patients - we showed that 8-isoprostane might represent a biomarker in NSCLC
sharing properties of response to chemotherapy as well as to the extent of the
disease. Prospective studies with larger numbers of patients are required as to
conclude in regards to 8 isoprostane representation as a valid biomarker of
evaluating oxidative stress in NSCLC population.
