Summary:
Polycystic ovary syndrome (PCOS) is one of the most common endocrinopathies in
women of childbearing age and occurs in approximately 6-12% depending on the
criteria used to diagnose the syndrome.
The most popular diagnostic criteria currently used are the criteria of NIH
(1990), the criteria of Rotterdam (2003) and the criteria of AES (2006). The
most commonly used criteria are those of Rotterdam.
The clinical syndrome typically occurs with oligomenorrhea or amenorrhea,
hyperandrogenism (clinical or biochemical) and / or polycystic morphology of
the ovaries sonographically.
Women with PCOS often present, obesity (peripheral type) glucose intolerance,
dyslipidemia, obstructive sleep apnea. Also, these women have an increased risk
of cardiovascular events and to develop later in life, type 2 diabetes,
metabolic syndrome and other complications. Women with PCOS may have as unique
symptom the infertility, which is mainly due to anovulatory cycles.
Women with PCOS and infertility who wish to become pregnant, after taking a
patient history and after excluding other causes of infertility, should be
treated in specialized centers by physicians with expertise in reproductive
endocrinology, and treated for ovulation induction with clomiphene citrate or
eventually undergo IVF or IVM.
Women with PCOS who are pregnant, have an increased risk compared with
the general population of early miscarriage, recurrent miscarriage, gestational
diabetes and fetal macrosomia and increased rates of caesarean section and
neonatal complications. There is also an increased chance for small for
gestational age infants (SGA). Finally, these women are at increased risk for
hypertensive disorders in pregnancy and preeclampsia.
