Summary:
Hoarding disorder (HD) has recently been included in Obsessive-compulsive spectrum disorders of DSM-5 as a distinct condition. Hoarding is characterized by a persistent difficulty in discarding possessions, strong urges to save items, and the accumulation of a large number of possessions leading to cluttered active living areas. The aim of this review is to summarize the current state of evidence regarding the comorbidity of hoarding. An electronic search was conducted for eligible studies in international search engines as PubMed, Scopus and Science Direct. Thirty studies of this search are included and criticized in the present review. Hoarding can occur in the context of a variety of psychiatric conditions. The first condition that is examined is that of obsessive–compulsive disorder (OCD). Although hoarding has frequently been considered a symptom (or symptom dimension) of OCD, many differences between HD and OCD are reported. Findings on neuropsychological deficits among hoarders and patients with OCD suggest that hoarding individuals exhibit dysfunction in the anterior cingulated cortex and prefrontal cortex, brain regions that differ from OCD patients. Compared with participants with OCD, those with HD often present higher percentages of attention deficit hyperactivity disorder (ADHD), social phobia and anxiety disorders as well as higher dysfunction and more symmetry obsessions with ordering arranging and counting compulsions. Clinical studies have demonstrated that hoarding often co-occurs with other mental disorders such as ADHD, social phobia, generalized anxiety disorder, major depression and post-traumatic stress disorder. Obsessive – compulsive, avoidant and schizotypal personality disorder, also show a high rate of co-occurrence with HD.
