Summary:
Objective: The main aim of this study is to review the till now available literature data on the role of minimally invasive retroperitoneal lymph node dissection in gynecological cancers by comparing the extraperitoneal versus the transperitoneal approach.
Methods: A systematic search of PubMed and Scopus library yielded 31 papers published between 2009 and 2017, which considered conventional laparoscopic, single-port laparoscopic and robot-assisted laparoscopic trans- and extraperitoneal pelvic and para-aortic lymphadenectomy in gynecologic cancers.
Results: The experience and applications of Laparoscopic (conventional, single-port and robot-assisted) Para-aortic Lymphadenectomy were analyzed. Outcomes were evaluated including age and BMI of patients, operative time, number of lymph nodes resected, hospital stay, estimated blood loss and complications (intra- and postoperative).
Conclusion: The extraperitoneal and transperitoneal approaches for laparoscopic and robotic aortic lymphadenectomy provide similar perioperative outcomes and nodal yields.
