Summary:
Pregnancy related cancer is making its appearance more often nowadays due to different socioeconomic conditions that have changed, as well as environmental factors. Breast cancer and cancer of the cervix are the most common malignancies associated with pregnancy and the puerperium. Other malignancies can be diagnosed as well such as lymphomas, thyroid cancer and melanoma. The diagnosis of cancer characterizes the pregnancy as high-risk. First of all the diagnosis is much more difficult due to the physiological changes of the pregnant woman, but also due to the limitations in diagnostic tests due to the pregnancy. The clinical findings and imaging testing are important in the diagnostic, but clinical suspicion must always be confirmed by histological examination.
The therapeutic approach during pregnancy is an important problem that should be designed by a multi-specialty group including the obstetrician - gynaecologist in collaboration with oncologist, cytologist, radiologist, radiotherapist and neonatologist in specially organized centres. The surgical treatment is safe during the pregnancy especially during the second and third trimester, depending on the location of the malignancy. There are cases that it can be performed even in the first trimester. Chemotherapy must be used in the second or the third trimester of the pregnancy, but radiotherapy must be used after delivery. This review of the main guidelines and data concerning cancer diagnosis and management during pregnancy aims to rephresh the current knowledge and sensitize healthcare professionals in order to achieve the best outcome possible for the mother and fetus.
