Summary:
According to the current MASCC-ESMO guidelines, a combination of a neurokinin-1 receptor (NK1RA), dexamethasone and a 5-HT3 receptor antagonist (5-HT3RA) is recommended to prevent cisplatin induced emesis. We performed a systematic review of randomized clinical trials Phase III (RCTs) that compared aprepitant + dexamethasone + 5-HT3RA versus dexamethasone + 5-HT3RA in patients who received the first cycle of cisplatin-based chemotherapy. The primary outcome was a complete response (CR), defined as non-vomiting and no rescue medication. Seven trials were eligible and complete response data were available from 7 studies (3574 patients).
The addition of aprepitant improves Complete Response in all phases: acute phase, late phase, but also in the total period, 69,92% vs. 52,41%. There was no significant heterogeneity between the clinical trials. In patients receiving cisplatin-based chemotherapy, the addition of NK1RA to dexamethasone and 5-HT3RA is associated with a clinically significant improvement in CR.
