Review of Adult and Pediatric Drug-Induced Sleep Endoscopy
Drug-Induced Sleep Endoscopy is used as an assessment technique to identify the anatomical segment responsible for obstructing the upper airway during sleep in patients with sleep related breathing disorders. This method is applied both in adult and pediatric population diagnosed with obstructive sleep apnea. The purpose of the study is the review of the articles about DISE. Scientific articles were retrieved via the electronic database Google Scholar. The articles included in the present study were published between 2015 and 2018. The accurate identification of the sites of obstructions contributes in diagnosis and in selecting the treatment method. In the case of surgical failure or failure to respond on continuous positive airway pressure (CPAP) DICE facilitates the selection of an alternative treatment method. The most common scoring systems are VOTE (velum, oropharynx, tongue base and epiglottis), the Pringle and Croft classification, the NOHL (nose, oropharynx, hypopharynx and larynx), Bachar and SERS (Sleep Endoscopy Rating Scale). The sedative agents that are used in DICE are propofol and dexmedetomidine, a propofol-remifentanil combination, a dexmedetomidine-remifentanil combination in adults and propofol alone or in combination with selvofluran and a combination of dexmedetomidine and ketamine in children. DICE is a reliable technique even when assessing inter-observer agreement between an experienced observer and a trainee. What emerges from this review is the necessity to establish a universally accepted scoring system and develop a standard protocol.
