Non-operative treatment of solid abdominal organ injuries
Summary:
It is widely known that on an annual basis in our country more than 2,000 deaths of young people are mostly due to injuries. Ιn the recent past τhe treatment of solid abdominal organs or vessel parts injuries was mostly surgical and accompanied with high rates of morbidity and mortality. This practice started being modified from 1980 onwards when it became evident that the presence of blood in the peritoneal cavity is not on its own an indication for conducting surgery. As a result, non-operative treatment was introduced.
The primary goal of this study is to evaluate the effectiveness of Non-Operative Treatment in traumatic injuries of solid abdominal organs in the concert of a tertiary urban Hospital reuters. The secondary goals involve evaluating parameters such as the kind of organ suffering, the age of the injured person, the degree of organ’s injury, the presence of coexistent conditions as well as of the ISS indicator, in successfully implementing of non-operative treatment of the injuries.
Μaterials and Methods: The database of the prospective study was drawn by injured people who were treated in the 1st Surgical Department of General Hospital of Nikaia ‘Agios Panteleimon’ from 2008 to 2015. It is consisted of 155 injured people with a wide and penetrating injury mechanism the treatment of which was based on a specific protocol that has been implemented from 2008.
Results: The database analysis has allowed reaching useful conclusions. The results have showed that non-operative treatment of the chosen injured patients (100 patients) was totally successful. Parameters such as age, anticoagulant treatment, coexistent ER, high degree of organ injury and increased ISS are not counter indications.
Conclusions: The Non-Operative Treatment of abdominal injuries is a safe and applicable method for the hemodynamically stable injured patients without acute abdomen symptoms. The method can be safely applied to centers that meet the necessary requirements.
