Summary:
Inflammation plays an integral role in the onset and progression of atherosclerosis. Endothelial dysfunction, the accumulation of oxidized lipids in the sub endothelial space and the induced involvement of innate and adaptive immunity are key points for disease progression. Increased inflammatory load, in the presence of systemic inflammatory diseases, such as rheumatoid arthritis and psoriasis, adversely affects the development of atherosclerosis, as it induce earlier onset, more severe progression and an increased number of complications. Existing assessment methods often fail to predict future cardiovascular risk in patients with underlying inflammatory conditions. The combined use of those methods, with the levels of pro-inflammatory cytokines such as IL-6, TNF-α and the more widely, best studied and easy to use in daily clinical practice hsCRP, could provide important data in assessing disease activity and control, as well as in recalculating future cardiovascular risk. Studies such as CANTOS, JUPITER, ATHEROMA and CIRT provide important evidence of the impact of systemic inflammation on the atherosclerotic process and its complications. The administration of antilipidemic agents of statins, with their pleiotropic anti-inflammatory effects, appear to play an important role in the reduction of future cardiovascular risk by providing better control over both cholesterol levels and systemic inflammatory diseases.
