Summary:
The rising prevalence of nonalcoholic fatty liver disease (NAFLD) is associated with the increasing «pandemic» of obesity. It has a broad spectrum, such as steatosis, steatohepatitis, fibrosis and cirrhosis. Current treatment of NAFLD is based on weight reduction. Bariatric surgery is the most effective treatment for morbid obesity and the co-morbidities that are associated with it. There is evidence that bariatric surgery improves histological and biochemical parameters of NAFLD. Weight loss and improvement of NAFLD are greater after Roux-en-Y Gastric Bypass (RYGB) than after other interventions. Among the mechanisms by which bariatric procedures may resolve NAFLD is weight loss, insulin resistance, changes of gastrointestinal hormones, inflammatory agents, bile acids and enteric microbiome. We reviewed the studies reporting the effect of bariatric surgery on the natural course of NAFLD and we discussed the possible mechanisms that may be involved in the alteration of the disease course. There are no randomized clinical trials evaluating the effects of bariatric surgery on NAFLD.
