Summary:
The aim of this study was to investigate the relationship between specific high-risk pregnancy categories and breastfeeding. Given the multiple benefits of breastfeeding for both the mother and the infant, a review of the existing literature has been carried out to highlight the problems faced by these high-risk pregnancy groups and to identify potential interventions in order to overcome them. Diabetes mellitus, either pre-existing condition or when it develops during pregnancy, obesity, premature birth and multiple pregnancy are high-risk pregnancies associated with additional breastfeeding challenges. Based on the literature review, despite the contradictory results of some studies, it has been shown that these pregnancy groups are associated with lower breastfeeding rates and shorter breastfeeding duration. This is usually attributed to problems relating to the onset of lactogenesis and also to the infant’s health and which may lead to a delayed onset or premature termination of breastfeeding. In addition, in these high-risk groups usually delivery is by caesarean section, in order to avoid any complications, thus making it even more difficult to initiate breastfeeding. Usually, cesarean delivery is accompanied by premature mother / infant segregation, decreased infant receptivity and poor milk supply, factors that result in reduced breastfeeding ability and shorter duration. Considering that breastfeeding seems to have even more benefits for these groups, both for infants and mothers, there is a need to conduct appropriate interventions and provide support to these high-risk groups in order to improve breastfeeding rates and duration.
