Summary:
The thyroid gland is the largest endocrine gland of the organism with a very important role in its function and homeostasis. It can develop functional disorders (hyperthyroidism and hypothyroidism) as well as morphological (nodules, goiter) and it counts the highest percentage of cancer among the endocrine system. The management of benign and malignant thyroid diseases accounts for conservative and surgical, and of course in case of malignancy, adjuvant and surgical exclusion (total thyroidectomy with or without neck lymph node dissection) is indisputable. During the last decades, the question of the thyroid microcarcinoma management (mostly papillary microcarcinoma) arises more and more, and especially concerning the cases where is accidentally found either in preoperative examination or in postoperative histological examination of a thyroidectomy sample for benign disease. While in the first case significant steps have been taken towards the establishment of guidelines, in the case of postoperative accidental detection of thyroid microcarcinoma, the management remains controversial as there is no system of risk assessment and prognostic factors. At the present time, correct and substantiated approach of the surgeon is required towards the decision of the surgical approach to the benign diseases, for protecting the patients from the need for further oncological or surgical treatment of a "thyroid incidentaloma" - microcarcinoma of the thyroid gland.
