Summary:
The naturally hypercoagulable state occurring during pregnancy anatomical changes and changes in the plasma volume are the main reasons for the increased risk of venous thromboembolism-VTE- during pregnancy and puerperium. This risk is particularly enhanced in the presence of thrombophilia and previous history of VTE. Other factors like hormone therapy, cancer, autoimmune diseases, heart failure, metabolic syndrome, sickle cell disease, air travelling, transfusions also increase the risk of VTE development. The cornerstone for treating and preventing VTE is administration of low molecular weight heparins (LMWH). The risk models used to stratify the risk for recurrence and based on the presence of factors such as previous V.T.E. (personal and familial history), thrombophilic status, co-morbities, and environmental factors and lead to decisions of dosing and duration of thromboprophylaxis. Treatment with LMWH is considered safe and effective with low incidence of adverse effects (bleeding, osteoporosis, HIT) and recurrence of VTE. The use of warfarin is restricted in pregnancy because of the coumarin related embryopathy. The use of direct oral anticoagulants DOACS is currently not recommended in pregnancy.
