Causes of delirium in Intensive care unit patients. A systematic review
Summary:
Introduction: Delirium is a syndrome that is often found in the field of intensive care units (ICU) and is associated with negative outcomes such as increased morbidity and mortality, increased costs of hospitalization, increased length of stay in the ICU. Knowledge of the causes leading to delirium in the ICU is essential for the immediate recognition of the syndrome and for its proper management.
Purpose: The aim of this study was to recognize causes and risk factors that lead to delirium in ICU patients.
Material and Methods: A search for studies published during the last five years (from January 1, 2015 to September 1, 2020) in English language in the PUBMED, MEDLINE, SCOPUS, Google Scholar databases. Original cohort studies and observational studies were selected for adult patients admitted to intensive care units for any reason, indicating causes and/or risk factors for delirium. Studies were excluded when title and summary were irrelevant, when minor patients were involved and when included patients were not ICU patients. The literature search took place from September 2020 to January 2021). The total number of studies included was 11.
Results: Causes that appear to be associated with the occurrence of delirium are age ≥65, comorbidities, alcohol abuse, hypertension, use of sedation, mechanical ventilation, prolonged hospitalization in the ICU, major surgery, high score in the APACHE II and SOFA scales.
Conclusions: In ICUs the incidence of delirium is very high. Given the high morbidity and mortality associated with the syndrome, it is useful to immediately recognize the risk of its occurrence in order to implement the necessary interventions both to prevent it and to reduce its severity and duration.
