Summary:
Background: Several studies have attempted to evaluate the association of breastfeeding (BF) with the risk of developing malignancies with the results so far being contradictory. The aim of this study was to summarize qualitatively and quantitatively the available data on this potential association.
Methods: A literature search of the PubMed was performed to identify available studies published between 01 January 2010 and 31 August 2021. A meta-analysis was performed using random effects models and estimating the odds ratio (OR) and 95% confidence intervals (C). Possible sources of heterogeneity were detected based on the Q-test and the I2 statistic. A publication bias test was also performed.
Results: A total of 16 studies, all case-controls, with 8,232 cases of children with malignancy and 17,473 controls were included in the meta-analysis. The summary risk assessments for pediatric cancer, regardless of type, in the comparison of any BF versus non- BF / occasional BF were 0.67 (95% CI: 0.46-0.98). The most protective association of BF was observed in the analysis by geographical area in which it was found that any BF against non- BF reduces the risk of malignancies by 72% only in Asia (OR 0.28, 95% CI 0.12, 0.65). 11 studies examined the correlation between BF and the development of childhood leukemia, however, no statistically significant relationship was found.
Conclusions: Our study supports the protective role of BF against the development of childhood malignancies. Nevertheless, further studies are needed, especially with prospective design, to confirm the negative association of breastfeeding and specific types of cancer.
