Outcome of cardiac surgery patients with diabetes mellitus - A comparative study
Summary:
Introduction: Insulin resistance and elevated blood glucose levels are common in hospitalized patients, regardless of a history of DM, especially in those undergoing coronary artery bypass grafting and aortic valve replacement procedures, increasing morbidity and mortality of patients Aim: The aim of this study was to investigate the outcome of diabetic patients after coronary artery bypass grafting and to compare it with non-diabetic patients. Material & Methods: 77 patients, who underwent coronary artery bypass grafting at the General Hospital "IPOKRATEIO", were the sample of the study. Data were collected from the patients' files during the period 15/02/2021-01/05/2022. Results: 64.9% of the sample was male and the mean age was 62.7±13.1years. Diabetes mellitus affected 42.9% and emergency admission was observed in 68.8% of participants. Comorbidities were reported by 61.0% and intraoperative complications occurred in 5.25%, and death occurred in 2.6%. More often emergency admission was seen in diabetic patients, p=0.005, and the same group most often reported positive family history of coronary artery disease,` p=0.005, more often experienced intraoperative complications, p=0.045, readmission to ICU, p=0.034, had older age, p=0.022; had abnormal glucose values preoperatively, on the first, second and third postoperative day, p<0.001, respectively; longer ICU length of stay, p=0.022; and lower ejection fraction at discharge, p<0.001. Conclusions: The glucose levels of diabetic patients undergoing coronary artery bypass grafting are observed at high abnormal levels compared to nondiabetic patients and are responsible for the hospitalization time, complications and overall outcome of these patients. Keywords: Coronary artery disease, diabetes mellitus (DM), coronary artery bypass grafting (CABG) surgery, outcome, hyperglycemia.
