Summary:
Bisphosphonates are widely used in the treatment of osteoporosis with proven effectiveness in reducing fracture risk. Even though bisphosphonates have a strong safety and tolerability profile, concerns have emerged about rare but serious side effects associated with their long-term use, including atypical fractures of the femur and osteonecrosis of the jaw. Due to the fact that bisphosphonates are incorporated into the bone and they continue to exert an antiresorptive effect even after its discontinuation, the idea of "drug holiday" has been introduced. The rationale of the “holiday” lies in the concept that the risk of adverse effects may be reduced whereas the patient continues to benefit from anti-fracture efficacy. However, questions remain on the optimal selection of candidates, the appropriate timing and duration of drug holidays as well as the monitoring during a drug holiday, all of which should be individualized weighing the risks and benefits.
