Summary:
Sympathetic renal denervation (RDN) has been proposed as a promising, minimally invasive method to help improve blood pressure control. Data from recent multicenter sham-controlled randomized trials provided important evidence that renal denervation is safe and efficacious, by achieving a clinically significant reduction in both office and 24-hour ambulatory blood pressure in hypertensive patients with and without antihypertensive therapy. In addition to lowering blood pressure, renal denervation has shown pleiotropic effects, most likely independent of the observed reduction in blood pressure, while recently data have also shown a positive impact on psychological disorders.
On the other hand, it is known that the Autonomic Nervous System (ANS) plays a dominant role in both hypertension and psychological disorders. Specifically, dysfunction of the autonomic nervous system is considered the strongest pathophysiological link between hypertension, depression and anxiety. Sympathetic stimulation is associated with increased blood pressure, while stressful life events are associated with depression and anxiety, which contribute to hypertension. It is therefore understood that restriction of sympathetic activation could have a significant positive effect on both control of blood pressure and management of psychological comorbidities.
Thus, the present MSc thesis (review of literature) summarizes the existing data, by paying special emphasis on the effect of renal denervation on stress management, anxiety disorders, depression and health-related quality of life in patients with resistant hypertension.
