Summary:
Introduction: The development of delirium in surgical patients is a
common postoperative complication.
Purpose: The purpose of this bibliographic review is to investigate the
causes and the risk factors that may be responsible for the phenomenon of
Delirium in patients that had preceded major surgeries. Also, the role of the nurse
in the assessment of that phenomenon and the procedures that can be applied
for the treatment of it. Finally, the validity of assessment tools that are used the
last years for the evaluation of the surgical patients.
Method: Greek and international relevant bibliography was searched in
medical databases, with key words concerning the definition, the types, the
causes, the risk factors, the screening tools and the ways of confronting delirium.
Results: The development of delirium is a frequent and serious
postoperative complication that brings upon a huge burden on the patients, the
family and in the health system generally. The pathophysiological causes of the
phenomenon are not fully comprehended. Two are considered to be the main.
The first one is the existence of infection on the central neural system and the
second one is neurochemical changes that blocks neurotransmission. The risk
factors that affect the intensity of the phenomenon are many and categorized in
preoperative, intraoperative and postoperative. It’s very important that all the care
team of the patient identifies the characteristics of the complication with the same
language. For this cause the screening tools of delirium contribute, which are
already numerically satisfied, simple in use and can be completed in few
minutes.
Conclusions: The frequency of delirium is quite high and focuses mostly
on the elderlies. With the help of assessment tools we can now detect in a
satisfying rate of success patients that maybe develop delirium and we can
organize a more targeted plan of care. Moreover, it is important that nurses are
trained, so that can use correctly the screening tools and identify promptly the
signs of the phenomenon.
