Summary:
Lung cancer remains the leading cause of cancer-related deaths worldwide. In the treatment of
lung cancer, radiotherapy plays a central role and serves as an excellent alternative for early-
stage patients who are either medically inoperable or refuse surgery. However, the use of
radiotherapy can be technically challenging due to the proximity of lung tumors to nearby
critical organs or structures. The most effective strategy to reduce radiation-induced toxicity is
to minimize unnecessary exposure of normal tissues by using advanced technology. Examples
from photon (X-ray) therapy include three-dimensional conformal radiotherapy compared to its
predecessor,
two-dimensional radiotherapy, and intensity-modulated radiation therapy compared to its
predecessor, three-dimensional conformal therapy. Proton therapy, as opposed to photon
therapy, has proven particularly effective for patients with NSCLC, providing significant
dosimetric advantages over traditional photon therapy. This is achieved by reducing
radiation to critical organs such as the heart, lung parenchyma, and esophagus, which in
turn can reduce acute and long-term toxicities in a population already susceptible to
pulmonary and cardiac damage. This review attempts to overview proton therapy
techniques, including related studies, clinical outcomes, and toxicities associated with
treatment, both in the early stages and in locally advanced stages of NSCLC.
