Summary:
COVID-19 has resulted in a pandemic resulting in significant morbidity and mortality. The COVID-19 pandemic has resulted in a large number of patients with moderate to severe acute hypoxemic respiratory failure (HARF). High-flow nasal cannula (HFNC) or non-invasive ventilation (NIV) may be a feasible option to avoid invasive mechanical ventilation and improve the clinical outcome of patients. HFNC is a non-invasive method of respiratory support that provides heated and humidified oxygen at a flow rate of up to 60 L/min, with accurate FiO2 titration. HFNC therapy has many clinical and physiological benefits in patients with ARF due to Covid-19. The mechanisms of clinical benefit are: 1. Humidification and warming of inspired air, 2. creation of positive airway pressures, 3. high and stable FiO2 value, 4. reduction of anatomical dead space, 5. reduction of the work of breathing and 6. good patient tolerance. HFNC is easy to implement and simple monitoring is required to monitor patients outside the ICU. Combining HFNC with prone positioning is associated with a better clinical outcome. Furthermore, HFNC is associated with a low risk of dispersal of the contaminant aerosol into the environment. HFNC reduces the risk of intubation compared with conventional oxygen therapy and is a reliable alternative to NIV in patients with ARF due to COVID-19.
