Summary:
Introduction: Preterm birth, i.e., birth that takes place in a period of less than 37 weeks of gestation, is the main cause of neonatal morbidity and mortality. It is a multifactorial disease; however, cervical abnormalities are a common feature that occurs in a large percentage of patients. Cervical insufficiency is an obstetric disease, manifested by repeated miscarriages in the second trimester of pregnancy or by spontaneous premature birth.
Purpose: The aim of the present study was the investigation of the available literature sources related to the interventional methods used to treat cervical insufficiency in pregnancy and the search for their effectiveness.
Methodology: PubMed and Google Scholar literature databases were searched for studies related to interventional methods used to treat cervical insufficiency. The filters used related to the subject matter of the studies, their publication date, and their language.
Results: From the search of the key words in the scientific databases PubMed and Google Scholar, 438 articles and bibliographic reviews emerged, which were investigated based on their content and the most appropriate ones were selected for presentation. The main interventional treatment presented in the literature includes cervical cerclage with Mcdonald, Shirodkar and transvaginal cerclage, while the main non-surgical treatments include vaginal progesterone and cervical pessary.
Conclusions: Based on the available studies, progesterone is the first line of defense used by the medical community to prevent preterm birth, while it tends to be used in combination with other therapeutic approaches. Cervical cerclage is an extremely effective treatment option for the treatment of cervical insufficiency and helps pregnant women deliver full-term fetuses. Although cervical pessary seems to be an effective option, further research is needed to ascertain its effectiveness.
