Summary:
Testicular cancer comprises the most common solid malignancy affecting males between the ages of 15 and 35. Germ cell tumors (GCTs) account for the vast majority of testicular cancer cases and are distributed between pure seminomas and nonseminomatous germ cell tumors (NSGCTs). Metastatic GCTs most frequently involve the retroperitoneal lymph nodes. Although the response to platin-based systemic chemotherapy is mostly excellent, retroperitoneal lymph node dissection (RPLND) is often required to remove the retroperitoneal disease, especially in the case of NSGCTs. Men with testicular cancer undergoing RPLND are at risk for sexual dysfunction and infertility due to loss of antegrade ejaculation resulting in deterioration of quality of life. Improvements in the understanding of anatomy and the pattern of metastatic nodal involvement resulted in the introduction of new surgical approaches with better postoperative results and lower morbidity. These approaches include the modified templates, the nerve-sparing RPLND and the minimally invasive techniques. The cornerstones of treatment include proper staging, a multimodal approach and an appropriate surgical technique.
