Summary:
Matrix metalloproteinases MMP-2 and -9 are a family of zinc-binding proteolytic
enzymes (gelatinases) that normally remodel the extracellular matrix (ECM).
The regulation of MMP activity is complex and occurs at multiple levels,
including interactions of secreted MMPs with tissue inhibitors of
metalloproteinases (TIMPs). In its inactive form, MMP-9 is complexed to TIMP-1,
whereas MMP-2 is complexed to TIMP-2 and the balance between MMPs and TIMPs
determines the degree of matrix degradation. The MMPs and the TIMPs seem to
control the degradation of ECM remodeling in many physiological conditions,
including ovarian follicular growth and ovulation. The ovarian tissue is in a
continuous morphologic change where some folliculars growth and others undergo
atresia . In the IVF treatment the ovarian stimulation needs pharmacologic
interference witch responses to the ovarian function that aimed to the
formation of multiple folliculars. The polycystic ovarian syndrome (PCOS) is
associated with the formation of multiple folliculars and ovarian cysts and
with ovarian atresia. Because of the increased levels of LH they produce
infertile ωocytes, whereas the abortion rates are higher. The present study
aimed to investigate the levels of MMP-2, MMP-9, TIMP-1 and TIMP-2 in the
follicular fluid of 39 women with PCOS and compare them with the levels found
in 56 normally ovulating women, all undergoing in vitro fertilization (IVF)
treatment and then correlate them with pregnancy rates in these two groups. All
the women received the same therapeutic long protocol ovarians stimulations in
the IVF treatment. None of them received the last three months any medication.
In their follicular fluids were detected in double the concentrations of MMP-2,
MMP-9, TIMP-1 and TIMP-2 using enzyme-linked immunosorbent assay ELISA. Women
with PCOS had significally higher concentrations of MMP-2 (p=0.02), MMP-9
(p<0.001), TIMP-1 (p=0.006) and TIMP-2 (p<0.001) in women with polycystic
ovaries than control healthy women. Women, with and without PCOS, who achieved
pregnancy had higher TIMP-1 levels compared to the non-pregnant ones in the
control group (p=0.01). MMP-2, MMP-9 and TIMP-2 levels did not have
statistically significant difference among the two groups.
