Summary:
Prostate adenocarcinomais the leading cause of cancer in men. Its incidence in
Europe is 79/100000 men. In this review the basic molecurar pathways, which
lead to cancerogenesis are demonstrated, such as the androgen receptor
regulated and PI3K/AKT pathway.Moreover the basic pathologic elements of
prostate adenocarcinoma are described. Gleason's score indisputable prognostic
value is also demonstrated. Main diagnostic tools necessary for accurate
prostte cancer diagnosis, such as biopsy, imaging modalities and PSA serum
values are also described with advantages and limitations of each
method.Furthermore, treatment strategies available for definitive treatment of
localized prostate cancer are explained such as active surveillance,
radiotherapy (EBRT and brachytherapy) and radical prostatectomy(open or
laparoscopically assisted).In what concerns the metastatic cancer treatment,
androgen blockade with its mechanism of action is outlined. Also, the side
effects and mechanisms of resistance of androgen therapies are
analyzed.Finally, the current treatment algorithm for castrate resistant
prostate adenocarcinoma (CPRC)is thoroughly described, including most recent
FDA drugs such as abiraterone acetate, cabazitaxel and enzalutamide.
