Summary:
Introduction: Arterial hypertension ,diabetes mellitus(DM) and metabolic
syndrome(MS) are prognostic factors of cardiovascular morbidity and mortality.
Purpose: The purpose of this study was to investigate the differences in blood
pressure profile between diabetic and nondiabetic patients with MS.
Materials and Methods: A total of 1159 patients with MS were enrolled in a
cross sectional study. Diagnosis of MS was based on The National Cholesterol
Education Program in Adult Treatment Panel Revised in 2005 criteria. Patients
were divided into two groups according to the presence of DM .Of the total
study population 320(27.6%) patients had DM and 839(72.4%) were not diabetics.
In all participants we measure office, home and ambulatory BP, we got complete
medical history, as well as anthropometric, echocardiographic variables and
laboratory blood tests.
Results: Diabetic patients with MS presented a mean age of 61.61±10.49 years
versus 57.82±11.36,p<0.001 in subjects with no DM ,greater frequency of
cardiovasclular events(14% versus 5%,p<0.001) and higher waist to hip
ratio(0.94±0.07 versus 0.92±0.07,p=0.003).Home diastolic BP(DBP) in nondiabetic
patients was 84.48±10.80 versus 81.81±10.29 in those with DM and office DBP
was 89.91±11.85 versus 86.45±11.41,p<0.001.Also significantly higher in
nondiabetic patients was 24h DBP(79.82±10.93 versus 75.69±10.72,p=0.001) and
day DBP(82.84±11.49 versus 78.21±11.08,p<0.001).Higher DBP had nondiabetic
patients during the night(72.03±11.14 versus 69.00±11.01,p=0.016).As far as
echocardiographic variables concerns patients with DM had fatter
interventricular septum(1.06±0.16 versus 1.04±0.16,p=0.044),posteriol wall(
1.05±0.15 versus 1.02±0.15,p=0.005),more elevated left ventricular mass
index(normalized by height elevated by power of 2.7)(45.50±12.68 versus
43.50±11.18,p=0.018) and higher early diastolic(0.77±0.18 versus 0.74±0.18,
p=0.032) and atrial velocities(0.88±0.19 versus 0.81±0.19,p<0.001).Concerning
medications, diabetic patients treated more often with antihypertensive
medications(85% versus 74%,p<0.001) and more specifically calcium channel
blockers(dihydropyridines) 43% versus 36%,p=0.025, beta blockers 36% versus
29%,p=0.028 and aldosterone receptor antagonists 3% versus 0.9%,p=0.014.
