Summary:
Aim
Assisted reproduction techniques (ART) account for 1-3% of all pregnancies
worldwide. The purpose of this study is the review of obstetric complications
associated with the implementation of ART methods and proposing strategies for
their prevention.
Material and Method
The review of existing literature included electronic libraries Pubmed,
Medline, Medscape and Uptodate. Emphasis was given to the recording of
complications in singleton pregnancies from ART and prevention methods for
their complications and especially multiple pregnancies.
Results
Obstetric and perinatal complications such as spontaneous abortion, ectopic
pregnancy, preterm labor, low birth weight and small for gestational age
newborns occur more frequently compared to pregnancies from spontaneous
conception. Leading obstetric complication are multiple pregnancies, while
there is an increased incidence of congenital anomalies, vertical transmission
of genetic diseases, increased perinatal mortality and maternal morbidity.
Processes such as blastocyst culture, in vitro maturation of oocytes and
preimplantation genetic diagnosis are responsible for an increased incidence of
premature birth and birth defects.
Basic strategies for reducing the risk of multiple pregnancies are optimization
of embryo selection, adoption of elective single embryo transfer and ovulation
induction primarily with clomiphene citrate or aromatase inhibitors and
subsequently with gonadotropins.
Conclusions
Pregnancies resulting from ART present a higher prevalence of obstetric
complications. A key priority is to reduce the rate of multiple pregnancies.
Further studies are needed to improve the assessment of higher quality embryos
and elective transfer of a single embryo.
