Summary:
The tenosynovitis is an inflammatory lesion of the tendon sheath surrounding
certain tendons and is a term widely used in any disorder of the tendon
sheaths. The tenosynovitis is divided into primary (idiopathic) and secondary.
The most common is idiopathic, caused by inflammatory changes with fibrosis and
thickening of the tendon sheaths which affects the tendon gliding and the
patient experiences the feeling of locking. The autoimmune rheumatic diseases,
which are known as diseases of collagen or connective tissue diseases are some
of the secondary causes of tenosynovitis. Inflammation is mainly due to
secondary arthritis or enthesitis as the spondyloarthropathies. The
tenosynovitis causes swelling, stiffness and tenderness on physical
examination. Diagnosis is by clinical examination. Ultrasound and MRI are
valuable tools that help in diagnosis. The inflammatory tenosynovitis should
always be diaforodiagnosed by infectious. Systemic treatment of any autoimmune
disease leads to remission of tenosynovitis. The ice, NSAIDs and splint are
widely used to relieve the symptoms. Local corticosteroid injection for acute
tenosynovitis is an alternative therapy. Finally in persistent cases
tendon-synovectomy is appropriate
