Summary:
LMWH’s antithrombotic effect is achieved through activation of antithrombin and
further inhibition of factor Xa. In special patient groups (obese, underweight,
pregnant and patients with renal insufficiency) the performance of anti-Xa
activity laboratory test is recommended, as dose modification may be required,
in contrast with the general population. However, inherited hematological
dysfunctions and, primarily, antithrombin deficiency (inherited or acquired)
may result in LMWH treatment failure if the treatment is not monitored. From
January 2013 until March 2016, anticoagulation treatment with LMWH was
monitored by clinical pharmacists at “Hygeia” hospital. Among others, (%)
antithrombin activity measurements were collected, which had been ordered for
patients with extremely low anti-Xa activity levels. Of the 441 patients, who
belonged to the general population, 143 (32%) had low and 56 (13%) high anti-Xa
activity. Regarding (%) antithrombin activity measurements in 115 subjects with
low anti-Xa activity, 101 (88%) of them were below the lower limit and only 12
(12%) were normal. Moreover, statistical analysis showed a significant negative
effect of influenza and treatment in an ICU department on (%) antithrombin
activity, which implies potential acquired antithrombin deficiency. Therefore,
anti-Xa activity monitoring in all patients at the initiation of LMWH treatment
should be recommended, in order to ensure treatment efficacy and safety.
