Aim
Almost one third of patients with lung cancer presents symptoms of obstruction of the central airways (CAO). The purpose of our study is to compare the treatment methods for obstructive endobronchial lesions and their efficacy.
Materials-Methods
We studied 30 patients who presented with symptomatic CAO between 2014-2016 in Oxford and Harefield Hospitals and we separated them into 3 study groups. Group A consisted of 10 patients who had endobronchial cryotherapy. Group B consisted of 10 different patients who had endobronchial treatment with Argon Plasma Coagulation (APC). Group C consisted of 10 different patients who had a combination of APC, at first, and then cryotherapy. The patients were matched by age, sex and underlying disease – causing CAO.
Results
After cryotherapy, the clinical symptoms were improved in all patients. However, for those who had APC, results were more direct. The chest X-ray post treatment was clearly improved and the previously atelectatic lung was expanding in all cases.
The treatments were efficient and successful for all the patients, while 30 days after the initial therapy we had no mortality. Only one of our patients who underwent cryotherapy presented haemorrhage that was controlled. The mean duration time of each therapy was 18.5, 11 and 20 minutes for Group A, B and C respectively. Each patient from Group A, B and C had on average 6.6, 5.3 and 5.3 sessions, respectively.
Conclusions
When it is feasible to apply both endobronchial cryotherapy and APC at the same session the results could be more impressive, offering more safety to our patients. The desired outcome can be achieved faster with less complication, within fewer sessions and can last for longer depending on the underlying disease.
