ABSTRACT
Background: Since 2008, many countries were confronted with the phenomenon of global economic crisis and its consequences in the fields of economy and society, and in the health sector. The increase in drugs’ consumption and public’s preference for expensive innovative drugs led to a subsequent rise of health expenditures, thus causing governments to turn their attention to the drug sector and more specifically to generic substitution. The introduction of generic medicines in national healthcare systems has been highly debated lately as an effective way to control and reduce pharmaceutical expenditures.
Aim: The aim of this study was to review developments in the introduction and use of generic medicine in international health systems in the period of recent economic crisis, analyzing the key market points and how it was affected by the implementation of economic austerity.
Method: The study was conducted through systematic literature review from 2007 to 2017 and the databases that were used were Pub Med, Google Scholar and websites of the Greek Ministry of Health, EL.STAT and OECD. Overall, 98 research studies have been found, from which 23 were selected. The selection criteria were: a) articles, which were primary studies, reviews or meta-analyses, b) articles available in full text, c) articles written in English or Greek and d) articles regarding people.
Results: The literature review shows that the introduction and use of generic medicines in international health systems during the economic crisis is a significant measure in reducing pharmaceutical expenditure, but also a low-cost means of healthcare provision. However, in several studies, doubt and suspicion by consumers

and health professionals were recorded, regarding the safety, efficacy and quality of generics.
Conclusion: As the need to control and reduce pharmaceutical expenditure is becoming increasingly urgent, governments all over the world are urged to develop policies promoting generics in parallel with maintaining the quality of healthcare and public health.
