Abstract
This study was conducted in the 1st Cardiology Clinic and in the homonymous Hemodynamic Cardiology Laboratory of the National and Kapodistrian University of Athens Medical School at Ippokrateio General Hospital of Athens.
The aim of the study was to investigate the effect of balloon aortic valvuloplasty prior transcatheter aortic valve implantation of a bioprosthetic valve, on myocardial necrosis.
In the present study, there were no statistically significant differences between patients who underwent balloon aortic valvuloplasty prior transcatheter aortic valve implantation (TAVI) and patients who underwent direct transcatheter aortic valve implantation regarding acute myocardial infarction, need for permanent pacemaker implantation, cardiac arrest during TAVI and death from any cause during TAVI and hospitalisation.
Troponin and creatinin kinase MB levels, that can indicate myocardial necrosis were recorded. It was observed that troponin levels increased statistically significant over the first two days after the TAVI versus pre-TAVI levels. However, balloon aortic valvuloplasty, has no statistically significant effect on troponin levels. Creatinin kinase MB levels were slightly increased on the day after TAVI, but not significantly versus pre-TAVI levels. Balloon aortic valvuloplasty seems that has no statistically significant effect on CK-MB levels.
In conclusion, we did not confirm our initial hypothesis that balloon aortic valvuloplasty prior transcatheter aortic valve implantation has an effect on myocardial necrosis.
