Summary:
The aim of this thesis is to review the literature regarding thrombosis related to central venous catheters among patients with cancer. Central venous catheters are broadly used in oncology; however they often lead to serious complications, including thrombosis, particularly of the upper extremity veins.
The incidence of catheter related thrombosis, symptomatic and asymptomatic, is not well-defined and is expected to change. Thrombosis is considered to be a multifactorial disease attributed to many risk factors related to the catheter itself and the cancer patient. The insertion of a central line causes endothelial damage and blood stasis, whereas cancer is a hypercoagulative state. As a result, the risk for thrombosis increases.
Diagnostic work-out is based on medical imaging and ultrasound is the method of choice. Conventional angiography is the “gold standard” method and it is used in selective cases. Modern imaging techniques seem to be very promising in the diagnosis of catheter related thrombosis.
Management is based on antithrombotic drugs and especially low molecular weight heparins. Duration of treatment should be at least 3-6 months and the catheter ought to remain in place except for specific cases. Universal use of pharmacologic thromboprophylaxis in patients with central lines is not recommended. Finally, it may be concluded that every cancer patient with central venous catheter should be approached individually.
