Residence time in ICU and factors affecting it in cardiac-operated patients
Summary:
INTRODUCTION: The incidence of postoperative morbidity has increased a fact which may lead to prolonged hospitalization to the patients who have undergone in cardiac surgery in the I.C.U and generally in the hospital.
AIM: Investigation of the factors affecting the residence time of the cardio-operated patients at the I.C.U.
MATERIAL AND METHOD: The studied sample consisted of 150 cardio surgery patients who were hospitalized in the I.C.U of a large private therapy in the city of Athens from May 2016 to September 2017. For data collection a special registration form with in formations coming from the medical records of patients was used. Data analysis was performed by the IBM SPSS 24.0(Statistical Package for Social Sciences).
RESULTS: Regarding to demographic characteristics and medical history of the studied sample, the majority were male (86%, n=117), with a mean age of 68 years and an average hospital stay of 2,32 days. The major causes of prolonged ICU stay was bleeding (36%, n=49), hypertasis (33,1%, n=45), arrhythmias (11%, n=15), and other causes (8,8%, n= 12).There was a statistically significant positive correlation between the age of the patients and their days of hospitalization at ICU. (p-value <0.05).
CONCLUSIONS: Patients with coronary artery disease who undergo coronary artery bypass and have a history of myocardial infarction appear to be more likely to prolong their hospitalization in the ICU. after cardiac surgery. Also, in most cases, preoperative patient pre-operative rates are related to post-operative treatment rates. In addition, a statistically significant positive correlation was found between the age of the patients and their days of hospitalization at the I.C.U as well as between the time of the mobilization of the patients after the surgery and the days of their hospitalization at the Meth. Finally, with the analysis of the data on the reasons for the extension of the hospitalization of the patients at the I.C.U. statistically significant differences were found in patients who experienced bleeding, arrhythmias, respiratory failure, infections, cardiac arrest and other causes (p-value <0.05). Patients who experienced some of the above causes remained at longer intervals in the ICU. Early recognition of these factors that may lead to prolonging patient hospitalization in the ICU may provide important information on how to improve perioperative care of the patient.
