Summary:
The aim of the present study is the review of the current literature in regard with the ultrasound guided block of the brachial plexus above the clavicle. There are 2 different types of blockade above the clavicle: the interscalene block and the supraclavicular block.
109 studies from the international bibliography have been reviewed, that referred to the techniques and approaches, the pitfalls, the complications and the newer data regarding the anaesthetic drugs used, as well as continuous perineural catheters.
The use of ultrasound for peripheral nerve blocks has caused a revolution at the field of Anaesthesiology and it has attracted the interest of many Anaethetists. Due to the application of ultrasound, the interscalene and supraclavicular blocks constitute ideal choices for reassurance of anaesthesia or /and analgesia at a multitude of surgical procedures of the upper arm. In comparison to the older methods (anatomic landmarks, peripheral neurostimulation) ultrasound imaging transfuses to these techniques easiness and safety. Therefore, in the context of a multimodal management of the perioperative pain, it has been observed an analgesia of better quality and longer duration, reduction of the risk of adverse effects related to opioid use and improved patient satisfaction.
