The therapeutic relationship between children with Autism Spectrum Disorder and their therapists as a means of maximizing the effectiveness of treatment outcome
Summary:
Building a therapeutic relationship is one of the most basic components for optimal therapeutic outcomes and thus it has drawn great research interest, as well as scientific controversy, over the years. Nevertheless, there is a need of studies assessing the role of therapeutic relationship between children with ASD and their therapists and moreover how this therapeutic relationship can influence the effectiveness of an ABA treatment for children with ASD. The purpose of the present study was to explore the concept of “therapeutic relationship” as it develops in interventions for children with ASD that are based on the principles of Applied Behavior Analysis. This was carried out by identifying and evaluating those treatment variables that are implemented by therapists in individualized sessions with children with ASD and also by evaluating the clinical picture and behavior of these children during those sessions. Four therapists were selected, two of which had extensive clinical experience, whilst the other two had minimal clinical experience. This was decided so that a comparison could be made between the therapists as to how they applied the various practices, both “desirable” and “undesirable”. The clinical picture of the two children with ASD was also evaluated during their therapeutic sessions with these four therapists. There have been several differences found between the “experienced” and “inexperienced” therapists as to how they implement the various treatment techniques and practices. The experienced therapists performed better in almost all treatment variables, in comparison with the inexperienced ones. At the same time, the two children with ASD had more correct responses, abstained from stereotypical and disruptive behaviors, engaged in on-task behavior and felt less discomfort and more happiness, when having therapeutic sessions with the experienced therapists. Descriptive statistics were used in this research. The reliability of the findings was tested by independent observers. The above findings of our research prompt the hypothesis that a child with ASD and his/her therapist can develop a special therapeutic relationship, which may be further shaped and established through the proper use and application of practices and techniques, that derived from Applied Behavior Analysis.
