Summary:
Introduction: Implementation of rehabilitation in mechanically ventilated patients has been demonstrated to be both safe and feasible and provide benefits in terms of physical and psychological function and assist with weaning from mechanical ventilation.
Aim: The purpose of this review is to describe respiratory physiotherapy over the last ten years, as it has been applied to patients in mechanical ventilation, due to a respiratory disease as cause of their admission to ICU. In addition, to determine which type of respiratory physiotherapy was most effective and how it affected the progression of the pulmonary disease.
Materials and Methods: A systematic review of papers, written in English and published from 2009 to 2019 was conducted. Search was performed on PubMed database with the keywords: "respiratory failure", "pulmonary diseases", "respiratory physiotherapy", "mechanical ventilation". Exclusion criteria: Studies involving pediatric patients, post-surgical patients or suffering from a neuromuscular disorder, studies referring to animal experiments or only abstracts.
Results: Only 13 of the scientific papers retrieved met the inclusion criteria, 4 of which were dedicated to respiratory rehabilitation in intubated patients due to a specific pulmonary disease while the other 9 were dedicated to respiratory rehabilitation for intubated, critically ill patients, with specification of the pulmonary disease on the patients’ demographic characteristics. Τhe most common cause of respiratory failure due to respiratory problems is pneumonia and then COPD exacerbation. Threshold inspiratory muscle training is more commonly used as part of respiratory physiotherapy, in combination with conventional physiotherapy.
Conclusions: Due to the small sample size and limited data, it was difficult to determine which type of respiratory physiotherapy was most effective and how it affected the progression of the pulmonary disease. Future research should identify this group of ICU patients and begin studying separately pulmonary diseases combined with specific types of respiratory physiotherapy.
