Venous thromboembolism in pregnancy
Summary:
Women present an increased risk of developing both arterial and venous thrombosis. Venous thromboembolism (VTE) is ranked among the first positions in terms of causing maternal mortality, especially in developed countries around the world. In Western Europe and the USA, VTE comprises a primary cause of death, with the majority of the cases being treated on time.

The danger of developing VTE during pregnancy is valid for all three trimesters, with the majority of the cases taking place during the first and third trimester, respectively. Maximum levels are reached during labour, as well as in the first weeks following the pregnancy. Risk factors leading to venous thrombosis are classified in factors related to pregnancy and factors related to maternity.

Pathogenesis of thrombosis is multifactorial and hence it is extremely difficult to assess the contribution of selected predisposing factors towards development of the process. Optimum dosing in terms of establishing a prophylactic strategy, has not yet been established, with the majority of the guidelines suggesting a single dose of low molecular weight heparin, adjusted to the body weight of the patient.

At the present time, it has not been possible to establish a uniform risk assessment tool for evaluating thrombosis and hemorrhage. It is very important for evaluation of thrombosis to take place as soon as possible, even before capture. Multicentre and well-designed clinical studies are needed in order to highlight the real contribution of administration of thromboprophylaxis during pregnancy.
