How the mode of birth influences the neonatal outcome and feeding in neonates who were born vaginally after caesarean section or through elective repetitive caesarean section.
Summary:
Purpose: The purpose of this study is to examine how the mode of birth influences the neonatal outcome and feeding.

Methods: It is a quantitative study. A questionnaire was distributed through the Internet to women who had one caesarian section and their next child was born vaginally or through repetitive caesarian section. There were open and close – ended questions that focused on breastfeeding or feeding with formula milk, the duration and cessation of breastfeeding, the mode and place of birth. The results were analyzed by SPSS program. Also Pubmed, Cochrane Database and Google Scholar were used to find studies that explain how the mode of birth influences the neonatal outcome and women’s intention to breastfeed.

Results: It is known that vaginal birth after caesarian section is a safe option for women who fulfill certain conditions. However the preceded caesarian is the basic reason the elective repetitive caesarian section is performed. In Greece VBAC is an option for women who can afford give birth in a private hospital. Most of the women who participated in the study were willing to breastfeed but those who had a caesarian section were more likely to give formula milk to the neonates or stop breastfeeding early. Caesarean section is connected with higher rates of neonatal and maternal morbidity and mortality compared to vaginal birth. Neonates who were born through caesarean section were more likely to have lower Apgar score, higher rates of admission to NICU, respiratory morbidity and hypoglycemia. These impacts depend on the week of pregnancy the caesarean is performed.

Conclusions: This dissertation revealed a number of conclusions that could lead to the improvement of the provided obstetric care. Health care providers should be well trained in order to inform sufficiently the women and be able to support their choice. It is essential for Greek hospitals to have protocols that will contribute to the improvement of the quality of midwifery care.
