Introduction:
Antibiotic overuse has led to the development of multidrug resistant microorganisms with an impact on patients' health, length of stay and hospital costs. Perioperative Antimicrobial Prophylaxis (PAP) is the administration of antibiotics before an operation to prevent surgical site infection. This study aims to describe current practices regarding perioperative antimicrobial prophylaxis at the “P. & A. Kyriakou” Children's Hospital in order to identify targets for improvement of judicious antibiotic use.
Methods:
All operations performed in the Surgery and ENT Departments of “P. & A. Kyriakoy” Children's hospital were examined retrospectively and prospectively for 2 months. Data recorded included patient demographics, type of operation and wound class, antibiotic agents administered along with time, dose and duration of regimen.
Results:
304 surgeries were recorded. 70,3% were boys. 21,5% of the procedures were urgent. The mean duration of the procedures was 69,7 minutes. Overall PAP use was considered appropriate for 45,4% of cases. At 31,9% of cases PAP was administrated without an indication.
As far as the cases who received PAP is concerned, 58,1% had no PAP indication. Adherence to guidelines for appropriate timing of first dose was 12,2%. In 70% of the cases the antibiotic administration was continued for more than 24 hours. The mean duration of the regimen was 6,2 days. Great variation was recorded regarding the choice of the regimen. First generation cephalosporin was never used. Oral antibiotics were administered to 60,5% of the patients either with or without intravenous regimens. Two or more antibiotics were prescribed to 15,7% of the patients. Full guideline adherence was recorded in 0,6% of the cases who received PAP.
Conclusions:
We identified injudicious antibiotic use regarding PAP for the indication (type of operation), time of initiation, duration and the choice of the regimen prescribed. These targets, along with the updated institutional guidelines of PAP, were used at a stewardship intervention in order to decrease unnecessary antibiotic use.
