Management of acute event during hemodialysis
Summary:
Introduction: Chronic kidney failure (CKD) is a complex disease affecting 1.2 million people worldwide. Its development is associated with a number of serious complications. Patients with CKD should be evaluated for the presence of these complications and receive optimal treatment to reduce their morbidity and mortality.
Aim: The investigation of acute events during hemodialysis and how to manage them.
Methodology: The study involved 30 patients (25 men) who undergo hemodialysis in the artificial kidney unit of the General Hospital of Athens "Ippokratio". The majority of patients were over 45 years old. For the data collection was used a registration form. The duration of the survey was from March to May 2020 (3 months). All patients underwent hemodialysis 3 times a week for 3 months. The inclusion criterion was end-stage kidney disease patients under artificial kidney with GFR <13 (glomerular filtration rate).
Results: 26.7% of patients reported diabetes as the cause of CKD. 80% of patients presented an event during hemodialysis, of which 50.7% had 1 event, 36.3% had 2, and 13.3% had 3 or more events. The most common occurrence of patients was muscle cramps (35.6%), followed by nausea, vomiting (23.3%) and hypotension (17.8%). Muscle cramps were treated with stretching exercises, nausea and vomiting with correction of hypotension and hypotension with trendelenburg position. Almost all patients improved after the management of acute events. Importantly was the fact that age and the causes of CKD did not affect the occurrence or the number of events in hemodialysis patients.
Conclusions: The present study recorded a high percentage of hemodialysis patients in whom acute events occur during hemodialysis, despite the fact that hemodialysis methods have improved and there is adequate and reliable management.
