Health related quality of life in children with surgically corrected congenital heart disease
Summary:
Introduction: It is a fact that compared to the healthy population, children with congenital heart diseases tend to have an impaired quality of life. As a result of the medical innovations that have taken place over the last decades, the survival rates of such patients have increased significantly and interest has now turned to investigating the parameters that affect their quality of life.
Aim: The aim of this study was to identify the factors that most affect the quality of life of children with surgically corrected congenital heart disease.
Method: A systematic review of articles published in the PubMed database from January 2015 to February 2020 was conducted using the following keywords: (Children*) AND (Congenital Heart Defect) AND (Quality of Life) AND (Surgery). The original search revealed 156 articles, of which 11 met the inclusion and exclusion criteria and were included in the review
Results: The review found that the factor that most affects quality of life is the type of heart disease; more specifically, children with a single ventricle have an increased risk of severe subsequent neurodevelopmental delay. The second factor that seems to affect patients' quality of life the most is the type of surgery and parameters related to the surgery, such as its duration, the time of cardiopulmonary bypass and the time of exclusion of the aorta. Moreover, the need for long-term hospitalization in the intensive care unit, and more generally in the hospital after surgery, would appear to have an impact on the subsequent neurological development of patients. In addition to the aforementioned factors, the family environment also seems to play a very important role, as parents with a higher educational and socio-economic level can better meet the needs of young patients, significantly improving their quality of life.
Conclusions: The better the diagnostic techniques and strategies become for dealing with children with congenital heart disease, the more urgent the need arises to implement quality of life assessment tools before, during and after treatment. Accordingly, those factors that have a negative impact can be identified in a timely manner, and targeted prevention and treatment strategies can be applied.
