Summary:
Bleeding Assessment Tools or BATs have been developed over the last twenty years in
order to assess and record the bleeding symptoms resulted from the fact that the routine
laboratory test with the PT-INR-Aptt, fibrinogen test is an hemostatic screening with
low sensitivity to detect hemostatic abnormalities.
Thus, for example, prolongation of Aptt may be associated with hemorrhagic disease or
with a thrombotic one. While VWD, which is one of the most common hemorrhagic
diseases of coagulation, it often manifests with normal values of APTT.
These gaps in the screening test, try to fill the ISTH-BAT (International Society on
Thrombosis and Hemostasis-Bleeding Assessment Tool) which has been validated and
widely used since 2010. It is a clear, understandable, detailed, and standardized
questionnaire designed to record bleeding in a stable way and with the minimum cost.
The goal of a BAT is to
1.improve diagnostic accuracy and thereby avoid unwarranted laboratory testing
2.predict the risk of bleeding in an individual patient
3.describe the symptom severity
4.inform treatment
After completing the questionnaire, the Bleeding Score is calculated by summing the
worst of each bleeding symptom. The worst episode of each symptom is graded
according to the bleeding score table. The final bleeding score is the total of all values.
This score gives valuable information about the patient's tendency for bleeding. Both
adult and pediatric BATs are available, as well as a newer combined tool developed by
the International Society of Thrombosis and Hemostasis (ISTH) in an effort to
consolidate the available tools.
In general, a high bleeding score (above the normal levels) indicates a type of
hemostatic abnormalities which it may manifest with an increased clinical propensity
for bleeding. The pre-operative identification of individuals at high risk of bleeding
during major elective surgery, is obviously important. Extensive hemostatic screening
is, however, expensive and may be inappropriate in low-risk groups.
In this study we tried to investigate the application the value and the usefulness of
ISTH-BAT in the patients of Ag. Savvas hospital who underwent various surgeries.
