Summary:
Introduction: Metabolic disorders that are expressed in organisms with septic shock also determine the selection of the appropriate nutrition protocol in patients with sepsis in intensive care units
Aim: Our aim is to summarize the current evidence for the way in which the application of different feeding models can affect the metabolic disorders manifested in sepsis. The study of the nutritional model used in septic patients and then the correlation with metabolic indicators can ultimately induce the application of the most eligible nutrition in such patients.
Material-Method: Systematic search was performed through the MEDLINE, Embase and Cochrane (Central) database aiming for randomized trials testing nutrition protocols in septic patients.
Results: In 5 studies reported the application of enteral, parenteral nutrition or a combination of both of them, with or without glutamine, in addition to a comparison of different intervals of their application. Patients' progress was assessed according to the SOFA and APACHE II scales as well as various metabolic indicators (plasma transferrin, nitrogen balance, creatinine/height index). The administration of intravenous plus through the intestinal tract glutamine was considered quite beneficial while there were controversial results as far as immediate initiation of enteral and combination with parenteral nutrition were concerned.
Conclusion: The ideal feeding route for septic ICU patients remains controversial. Nowadays, the enteral tract is recommended in combination with additional parenteral nutrition, in order to achieve the goal of protein-calories, to take advantage of the enteral nutrition on the intestinal mucosa and to achieve metabolic yield in septic patients.
