Summary:
Diabetes mellitus is a metabolic disease caused by either a disorder of insulin synthesis, or a disorder in the action of insulin, or both. It is the leading non-communicable disease in the developing world, causing 4.8 million deaths and disability in 371 thousand people each year. The incidence of the disease has increased in recent years and the age of onset is increasing. It is estimated that there are 28 million women of childbearing age worldwide with diabetes. Most of them suffer from type 2 diabetes.
Diabetes mellitus during pregnancy is associated with increased risk only for the mother and for the fetus and is divided into two categories: the pre-existing diabetes type I or II and gestational diabetes mellitus (GDM), which is a disorder of glucose tolerance, with onset or first recognition during pregnancy. The diagnostic approach of GDM is done into two steps and involves the detection test (screening test) and diagnostics (diagnostic test) applied to women who exceeded the set limit of glucose detection during testing.
The newborn of diabetic mothers is at increased risk for complications such as congenital malformations, spontaneous abortions, intrauterine death, macrosomia, hypoglycemia, hypocalcemia, respiratory distress, myocardial compassion and jaundice. Several perinatal complications due to the effects of high levels of maternal glucose and can be avoided with the right setting at the time of conception and pregnancy. Early detection and diagnosis, systematic monitoring, maintaining good metabolic control and the identification of risk factors for mother and infant have resulted in a dramatic improvement in the prognosis of the GDM.
